Job Description

We are looking for a dedicated PRN Social Worker to join the team at LifeBridge Health in Baltimore, MD. As a valued member of the hospital staff, you will be responsible for providing clinical social work services to patients and their families, addressing various challenges including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients/residents and their families.
• Offer counseling and support for individuals facing medical diagnoses and treatments.
• Assist with discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
