SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will involve addressing various challenges, including adjustment to institutional living, financial concerns, family relationships, and counseling related to medical treatment and end-of-life care.

Responsibilities

• Provide clinical social work services to patients and families.
• Assess and address common psychosocial issues.
• Offer counseling and support for patients facing end-of-life care.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
