Job Description
We are currently looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life issues.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support regarding medical procedures, discharge planning, and end-of-life care.
- Collaborate with healthcare teams to address patient needs.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
