SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of common challenges such as adjustment to institutional living and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Address financial concerns, family relationships, and other issues affecting patients' well-being.
• Assist in discharge planning and facilitate support resources for patients and families.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree and licensure as LBSW/LMSW/LGPC.
• 1-3 years of relevant experience.
• Strong knowledge of social work theory and practice.
• Excellent interpersonal and communication skills.
