Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various challenges such as adjustment to institutional living, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
• Provide counseling related to death and dying.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Professional knowledge in social work practices.
