SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team on a PRN (as needed) basis at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a variety of common issues related to institutional living, financial concerns, family relationships, medical understanding, aging, discharge planning, and counseling for end-of-life situations.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues related to patient and family well-being.
• Collaborate with interdisciplinary teams to support patient care.
• Conduct counseling related to end-of-life and palliative care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
