Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health's Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common challenges such as institutional living adjustment, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide intervention strategies.
• Assist patients and families with discharge planning and coping mechanisms.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
