SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and emotional support.
• Assist with discharge planning.
• Collaborate with interdisciplinary teams.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as an LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
