Job Description
We are looking for a dedicated PRN Social Worker to join the team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families. Your responsibilities will include addressing various common problems such as adjustment to institutional living, financial concerns, family relationships, discharge planning, counseling related to death, and more.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Address a range of issues including aging, financial concerns, and family relationships.
- Work collaboratively with a multidisciplinary team to provide optimal care.
- Assist with discharge planning and support patients and families through difficult situations.
Requirements
- Bachelor's degree with 1-3 years of experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of clinical social work theory and practice.
- Ability to work with compassion and empathy in challenging situations.
