Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment issues, financial concerns, and end-of-life planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Assist with issues related to adjustment, financial concerns, and family relationships.
• Provide counseling and support for end-of-life planning.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree with LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in a healthcare setting.
• Familiarity with social work theory and practice.
• Strong communication and interpersonal skills.
