SOCIAL WORKER PRN

Job Description

We are currently looking for a professional Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. You will address a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients/residents and families.
• Offer counseling and support regarding various emotional and practical concerns.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a similar role.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
