Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. The position involves providing clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services to patients and families.
• Assess and provide support for common challenges related to medical procedures, aging, and end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Engage in discharge planning and counseling.

Requirements

• Bachelor's degree in social work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong knowledge of social work theory and practice.
