SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a member of the social work team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common problems related to institutional living, family dynamics, medical procedures, aging, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Assist in discharge planning and support patients in adjusting to institutional living.
• Address financial concerns and facilitate communication regarding medical treatments and diagnoses.
• Offer emotional support and guidance in situations related to death and dying.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Working knowledge of social work theory and practice.
