Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. This position involves addressing issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems related to adjustment, financial concerns, family relationships, aging, discharge planning, and grief counseling.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
