SOCIAL WORKER PRN

Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying. This position requires 1-3 years of experience and relevant licensure (LBSW/LMSW/LGPC).

Responsibilities

• Provide psychosocial assessments to patients/residents and their families.
• Address common problems related to adjustment, financial concerns, family relationships, and end-of-life counseling.
• Participate in discharge planning and help patients understand medical procedures and treatments.
• Collaborate with interdisciplinary teams to support patient care.

Requirements

• Bachelor’s degree or equivalent in social work or related field.
• 1-3 years of experience in social work.
• Licensure as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
