Job Description

We are looking for a dedicated PRN (as needed) Social Worker to join the team at LifeBridge Health. Based in Baltimore, MD, you will provide crucial clinical social work services and psychosocial assessments to patients and their families, addressing a range of challenges frequently encountered in a healthcare setting. Your responsibilities will include assisting patients with institutional living adjustments, financial issues, family dynamics, medical understanding, aging concerns, discharge planning, and end-of-life counseling.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
