Care Manager RN - Pediatrics

We are looking for a skilled Care Manager RN to join our Medical Management/Health Services team at Superior HealthPlan. In this role, you will assess, plan, and coordinate medical and support services for select members to ensure quality, cost-effective care. You will work as a hybrid position in SDA Travis County, Texas, and must be a licensed RN residing in Texas.

Responsibilities

• Assess members' health status, treatment plans, and goals.
• Develop care plans and interventions to meet member's needs.
• Coordinate services across healthcare providers and community resources.
• Provide patient and provider education.
• Monitor and report on care management quality concerns.

Requirements

• Graduate from an Accredited School of Nursing.
• Bachelor’s degree in Nursing preferred.
• 2+ years of clinical nursing experience.
• 1+ years of case management experience in a managed care setting.
• Current state RN license.

Compensation

Pay Range: $66,100.00 - $118,900.00 per year. Centene offers competitive pay, health insurance, 401K, stock purchase plans, tuition reimbursement, paid time off, and flexible work options.
