Job Description
We are currently looking for a dedicated Inpatient Case Manager (RN) to join LifeBridge Health's dynamic team at Northwest Hospital in Randallstown, MD. This is a full-time position with a weekend commitment, offering day and evening shifts from 8:30am to 7:00pm. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to facilitate discharge planning and interventions.
Responsibilities
- Coordinate discharge planning efforts with a focus on patient wellness and improved care outcomes.
- Ensure efficient utilization of health services and minimize payment denials for patients with complex health needs.
Requirements
- Basic professional knowledge with a Bachelor's degree; familiarity with specialized field theory and practice.
- BSN required from CCNE accredited schools, with exceptions for specific LBH candidates.
