Job Description
Liberty Hospice is currently seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our team on a full-time basis in Cumberland County. As an RN at Liberty Hospice, you will play a crucial role in providing exceptional care to our hospice patients, emphasizing strength, dignity, and compassion.
Responsibilities
- Apply a working knowledge of nursing theories and concepts.
- Coordinate and document patient care to meet reimbursement guidelines.
- Collaborate with patients, families, physicians, and the Hospice team to ensure comprehensive care.
- Communicate effectively with patients, families, and agency personnel.
- Participate in IDT meetings and quality reviews as scheduled.
Requirements
- Current RN licensure in the state of practice.
- Minimum of two years of clinical nursing experience with at least one year in hospice nursing preferred.
- Knowledge of home health and hospice care programs and the nurse's role in patient care.
- Proficiency in nursing principles and practices, agency policies, and reimbursement guidelines.
- CPR certification and 12 hours of continuing education annually.
- Willingness to travel based on agency needs and work weekends, evenings, and holidays as required.
