INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient health service utilization, and minimizing payment denials for patients with complex health needs.

Responsibilities

• Provide discharge planning coordination in collaboration with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patients with complex health needs.

Requirements

• Basic professional knowledge with equivalent to a Bachelor's degree.
• BSN required from CCNE accredited schools.
