Job Description

We are looking for a dedicated Care Manager to join the Agency on Aging of South Central CT. In this role, you will be responsible for assessing clients' needs and coordinating the delivery of essential care services. Your primary tasks will involve completing health assessments, educating clients about available resources, assisting with program applications, and monitoring the effectiveness of care plans. You will also be required to maintain accurate documentation to comply with agency guidelines and meet funding source deadlines. Responsibilities

  • Complete health assessments and necessary paperwork for continued services.
  • Educate clients on available resources and program services.
  • Assist in completing applications for supportive services and programs.
  • Monitor and adjust care plans as needed.
  • Maintain accurate and timely documentation.
  • Prepare reports and meet critical deadlines.

Requirements

  • Bachelor's degree in health, social work, gerontology, nursing, or related field.
  • Minimum of one year of experience working with older adults or individuals with disabilities.
  • Excellent interpersonal and organizational skills.
  • Knowledge of community resources and documentation requirements for state-funded programs.
  • Bilingual skills are a plus.
