HOSPICE MEDICAL SOCIAL WORKER

Job Description

Join Liberty Healthcare Management as a Hospice Medical Social Worker and make a difference in the lives of patients and families facing terminal illness. In this role, you will provide direct patient care services, conduct clinical assessments, develop individualized care plans, and collaborate with other healthcare professionals to ensure optimal care delivery. Additionally, you will offer bereavement support and educate patients and families on available community resources.

Responsibilities

• Provide direct patient care services and document care data.
• Conduct clinical assessments and develop individualized care plans.
• Identify social, emotional, financial, and environmental factors affecting treatment.
• Coordinate care with agency personnel and other services.
• Assist with homecare visits and provide bereavement support.
• Educate patients and families on community resource planning.

Requirements

• Master's degree in Social Work from an accredited school.
• Minimum of one year of practical experience, preferably in home health/hospice care.
• Strong interpersonal and communication skills.
• Willingness to travel as necessary.
• Completion of a minimum of twelve hours of continuing education annually.
