Job Description

We are looking for a dedicated and skilled Licensed Practical Nurse to join Pediatric Associates of Greater Salem Inc as the Care Team Nurse. In this vital role, you will work closely with the Pediatrician, Nurse Practitioner, and Nurse Manager to ensure the smooth operation of clinical activities, manage high-risk patients, provide education to patients and families, deliver direct care and conduct laboratory services, as well as triage both over the phone and in person.

Responsibilities

  • Demonstrate proficiency in infection control practices including standard precautions.
  • Provide age-appropriate care for pediatric patients, utilizing knowledge of medical equipment and instruments.
  • Assess and address the physical, emotional, and socio-economic needs of clients and families, making referrals when necessary.
  • Offer urgent triage care in-office and via telephone, as well as manage crises effectively.
  • Document nursing care accurately in the EMR system, covering triage, education, medication, reconciliation, and care planning.
  • Facilitate communication among patients, families, physicians, and other healthcare professionals.
  • Administer vaccinations, medications, and perform in-office procedures efficiently.
  • Follow up with patients post-ER or hospital discharge to prevent readmissions and complications.
  • Keep up-to-date with pediatric-focused education and training.
  • Adhere to practice policies and procedures, meeting physical demands of the role.

Requirements

  • Strong written and verbal communication skills with excellent organizational abilities.
  • Capacity to work independently and collaboratively within a care team environment.
  • Ability to handle confidential medical information in compliance with HIPAA regulations.
  • Proficiency in EMR systems, Microsoft Office, and email communications.
  • Graduation from an accredited nursing program.
  • Current licensure to practice as a Licensed Practical Nurse (LPN) in Massachusetts.
  • Maintenance of Basic Life Support (BLS) certification and Continuing Educational Units (CEUs) as required.
  • Adherence to American Nursing Association Standards of Practice.
