Primary Care Physician (Bilingual Spanish)

Job Description

Join Esperanza Health Centers as a Primary Care Physician (Bilingual Spanish) and make a difference in the lives of older adults in our Program of All-Inclusive Care for the Elderly (PACE). As part of our dynamic team, you will provide primary medical care services, emphasize illness prevention, promote healthy lifestyles, and participate in the care coordination of PACE participants.

Primary Duties and Responsibilities

• Manage primary care practice in PACE Center and participants' homes.
• Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans.
• Contribute to quality improvement activities and ensure sound utilization management.
• Maintain communication with partner facilities to ensure continuity of care.

Requirements

• Bilingual in English and Spanish.
• Board certification in primary care; specialization in geriatrics preferred.
• Illinois physician license, valid driver's license, and current automobile insurance.
