Job Description

We are currently seeking a dedicated and experienced Home Care Registered Nurse to join our team at Liberty Home Care. As a Registered Nurse, you will play a vital role in providing professional nursing services to our patients within Harnett County, promoting positive outcomes through collaborative care practices. Your responsibilities will include actively participating in care planning, accurate documentation of services, and fostering effective interpersonal relationships.

Responsibilities

• Actively participate in the interdisciplinary care planning process.
• Provide professional nursing services in alignment with established care plans.
• Promote favorable outcomes through collaborative practice patterns and cost-effective resource utilization.
• Document accurately and thoroughly to demonstrate evidence of appropriately delivered home health services.
• Demonstrate positive and effective interpersonal relationships.
• Show personal accountability for work assignments and professional growth.

Requirements

• Successful completion of nursing education and current RN licensure.
• At least one year of clinical nursing experience in acute care or long-term setting.
• Home health experience preferred.
• Knowledge of medical-surgical nursing principles and clinical nursing skills.
• Ability to communicate effectively and use computer applications.
• Problem-solving, critical thinking, and reasoning skills for patient care planning.
• CPR certification.
