Job Description
As a HOSPICE CLINICAL MANAGER - RN at Liberty Hospice, you will play a crucial role in coordinating case management, providing direct patient care, and supervising Care staff. Join our compassionate team dedicated to delivering state-of-the-art care to patients in Cumberland County.
Responsibilities
- Coordinate case management of all patient services.
- Supervise assigned Care staff and ensure compliance with regulations.
- Provide direct patient care including case management and Oasis care planning.
- Assist Administrator as needed for efficient service delivery.
Requirements
- Current RN licensure in North Carolina.
- Minimum three (3) years of clinical nursing experience, hospice experience preferred.
- In-depth knowledge of hospice/homecare regulations.
- Strong critical thinking and communication skills.
- CPR certification and proficiency in computer applications.
