Social Worker

Job Description

Northwest Kidney Centers is looking for a dedicated Social Worker to join our team on a full-time, variable basis. This role involves providing social work intake, patient advocacy services, and coordinating social services with various stakeholders. The Social Worker will be responsible for assessing patients' social service needs, developing personalized care plans, and ensuring quality care delivery in compliance with ESRD Conditions of Coverage.

Responsibilities

• Provide patient education, counseling, and support services.
• Work collaboratively with interdisciplinary teams to enhance patient care.
• Participate in Quality Assessment Performance Improvement initiatives to optimize patient care.

Requirements

• MSW degree and current WA Social Worker License.
• Prior experience in medical social work is preferred.
• Possession of a valid WA Driver's license, reliable transportation, and car insurance.
