HOSPICE SOCIAL WORKER- WEEKEND WORK ONLY

Job Description

We are looking for a dedicated Hospice Social Worker to join our compassionate team at BRIDGING LIFE HOSPICE HOME HOSPICE - WEST. This is a part-time position for weekends, with day and evening shifts available (8:00am-8:30pm). As a Hospice Social Worker, you will play a vital role in providing professional and family-oriented services to individuals in the home setting. Your responsibilities will include assessing patients' and families' needs, formulating care plans, offering counseling services, and coordinating referrals for additional support services.

Requirements

• Master's Degree in Social Work.
• 2-4 years of clinical experience, with preference for community health settings.
• LCSW or LCPC certification.
• Professional licensure in Maryland and Pennsylvania.
