Job Description
We are looking for a dedicated Inpatient Case Manager - Social Worker (LMSW / LCSW-C) to join our team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial interventions for high-risk inpatients. Your work will focus on promoting patient and family wellness, improving care outcomes, and facilitating access to hospital and community resources for patients with complex health needs.
Responsibilities
- Provide patient and family advocacy within an inpatient setting.
- Coordinate discharge planning and facilitate access to resources for high-risk patients.
- Collaborate with the clinical team and medical providers to ensure comprehensive care.
- Conduct psychosocial interventions to support patient well-being.
Requirements
- Master's in Social Work required.
- LMSW required; LCSW/LCSW-C preferred.
- 3-5 years of relevant experience.
- Maryland Social Work License per level of education.
- Strong verbal and written communication skills.
