Job Description
We are looking for a dedicated Inpatient Case Manager - Social Worker (LMSW) to join our compassionate team at LifeBridge Health. As an Inpatient Social Worker, you will collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your role will focus on promoting patient and family wellness, improving care outcomes, and facilitating access to appropriate hospital and community resources for patients with complex health needs.
Responsibilities
- Provide patient and family advocacy.
- Coordinate discharge planning.
- Offer psychosocial interventions for high-risk inpatients.
- Promote patient and family wellness and improved care outcomes.
- Facilitate access to hospital and community resources.
Requirements
- Master's in Social Work Required.
- 2 years of hospital social work/community social services agencies experience preferred.
- LMSW required; LCSW/LCSW-C preferred.
- MD Social Work License per level of education.
- Effective verbal and written communication skills.
