Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health. As a full-time Social Worker based in Levindale Hospital, Baltimore, MD, you will provide clinical social work services to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life support. Responsibilities
- Conduct psychosocial assessments and provide counseling to address patient and family needs.
- Assist with discharge planning and support patients facing difficult medical decisions.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
