Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a variety of common challenges related to institutional living, financial concerns, family relationships, aging, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address various issues.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with other healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) certification required.
