INPATIENT CASE MANAGER (RN)

Job Description

We are currently seeking a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will work collaboratively with the clinical team and medical providers to facilitate discharge planning coordination and interventions for a patient population with complex health needs. The primary goal is to promote patient wellness, enhance care outcomes, optimize health service utilization, and reduce payment denials.

Responsibilities

• Coordinate discharge planning in partnership with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Optimize health service utilization and minimize payment denials.
• Collaborate with a multidisciplinary team to ensure comprehensive care management.

Requirements

• Basic professional knowledge with a Bachelor's degree or equivalent.
• BSN preferred; ADN required.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
