INPATIENT CASE MANAGER (RN) - PRN

Job Description

We are looking for an experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical providers to deliver discharge planning coordination and interventions. Your role will focus on promoting patient wellness, improving care outcomes, optimizing health service utilization, and minimizing payment denials among a patient population with complex health needs.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN preferred.
• 3-5 years of related experience.
• American Heart Association CPR Certification.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
