Job Description

Summary
The Inpatient Care Manager collaborates with the clinical team and medical provider to coordinate discharge planning and interventions. They focus on promoting patient wellness, improving care outcomes, ensuring efficient health services utilization, and minimizing payment denials in a patient population with complex health needs.
Requirements
- Basic professional knowledge with a Bachelor's degree; specialized field experience.
- BSN preferred, ADN required.
- 3-5 years of related experience.
