INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will work collaboratively with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient health services utilization, and reducing payment denials.

Responsibilities

• Provide discharge planning coordination and intervention for inpatient cases.
• Promote patient wellness and improved care outcomes.
• Efficiently utilize health services while minimizing payment denials.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• BSN required from CCNE accredited schools.
• Experience in a specialized field is a plus.
