RN Care Coordinator, Ambulatory Care

Job Description

We are looking for a dedicated RN Care Coordinator to join our team at Denver Health. As the RN Care Coordinator, you will be responsible for coordinating patient care, ensuring quality outcomes, and collaborating with multidisciplinary teams and patients/families. You will work in the Denver Sexual Health Clinic, providing essential services to an underserved community.

Responsibilities

• Use the nursing process to establish patient-centered goals and plans
• Coordinate the delivery of care within the clinic and across healthcare settings
• Advocate for patient benefits and eliminate barriers to care
• Maintain a positive and inclusive work environment

Requirements

• Associate's Degree in Nursing
• 3 years of clinical experience
• Active RN license and BLS certification
• Strong communication and problem-solving skills
• Proficiency in Microsoft Office and electronic health records
