Job Description
We are looking for a dedicated Complex Care Social Worker - LMSW to join our team at LifeBridge Health. This full-time position based in Baltimore, MD, offers an opportunity to make a difference in the lives of challenging patients by facilitating discharge planning and achieving goals for length of stays. The ideal candidate will work closely with the interdisciplinary team to address barriers hindering effective discharges and implement early interventions for timely and cost-effective patient care.
Responsibilities
- Facilitate discharge planning for difficult to place, high-cost, and escalated patients.
- Utilize a multi-step escalation protocol to identify barriers to discharge.
- Assume responsibility for escalated patients and implement standards for discharge planning.
- Monitor difficult to place and high-cost patients for early intervention opportunities.
Requirements
- Master's degree in a related field.
- Licensed Master Social Worker (LMSW) certification.
- Professional knowledge and experience in discharge planning.
