Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer support to patients/residents and families facing various challenges. This role involves assisting with adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Key Responsibilities
- Offer clinical social work services and psychosocial assessments to patients/residents and their families.
- Address common issues such as adjustment to institutional living, financial concerns, family relationships, and more.
- Collaborate with healthcare teams to develop effective care plans.
- Provide counseling and support related to end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Compassionate and empathetic approach to patient care.
