Job Description
We are seeking a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patients through difficult life transitions.
- Collaborate with healthcare teams to ensure comprehensive care for all patients.
Requirements
- Bachelor's degree in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
