Job Description - Social Worker
We are looking for a dedicated Social Worker to join LifeBridge Health, a leading health care provider in Maryland. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide support to patients and families.
- Assist with discharge planning and facilitate coping mechanisms.
- Collaborate with healthcare teams to address patients' social and emotional needs.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Strong interpersonal and communication skills.
