Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support to patients and families.
- Implement discharge planning.
- Collaborate with healthcare professionals to ensure holistic care.
Requirements
- Bachelor's degree in Social Work (BSW).
- Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
