Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health, one of Maryland's largest healthcare providers. As a crucial member of our team, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Assess and address common problems related to emotional, financial, and social aspects of patient care.
- Counsel patients and families on issues such as aging, discharge planning, and end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification is a plus.
- Demonstrated experience in clinical social work or related field.
- Compassionate, empathetic, and patient-centered approach to care.
