Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a wide range of challenges. This role involves assisting with adjustment issues, financial concerns, family relationships, aging-related changes, and end-of-life counseling.
Responsibilities
- Providing clinical social work services to patients and families.
- Assessing and addressing common psychosocial problems.
- Collaborating with healthcare teams to ensure holistic patient care.
- Assisting with discharge planning and counseling related to death and dying.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong interpersonal and communication skills.
