Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling. Responsibilities
- Provide clinical social work services to patients and families.
- Address challenges related to aging, discharge planning, and end-of-life counseling.
- Collaborate with healthcare teams to support patients' well-being.
- Conduct psychosocial assessments and develop care plans.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Experience in a hospital setting is a plus.
