Job Description
We are looking for a dedicated Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services to patients, residents, and their families, addressing a range of psychosocial issues such as adjustment to living in an institutional setting, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients and families with discharge planning and coping with medical diagnoses.
- Collaborate with multi-disciplinary teams to address patient needs.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Prior experience in a clinical setting is a plus.
