Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a vital member of our team, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges and concerns they may face.
Responsibilities
- Offer counseling and support to individuals and families dealing with institutional living, financial issues, family dynamics, medical procedures, aging-related changes, and end-of-life matters.
- Conduct thorough psychosocial assessments to identify needs and develop care plans.
- Collaborate with healthcare professionals to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) credential.
- Excellent communication and interpersonal skills.
