Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a full-time member of our team, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist with discharge planning and end-of-life care.
- Collaborate with multidisciplinary teams to support patient care.
- Advocate for patients' rights and well-being.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Prior experience in a healthcare setting is a plus.
