Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health in Baltimore, MD at Levindale Hospital. As a member of the Allied Health team, you will provide clinical social work services to patients/residents and families, addressing various challenges such as adjusting to institutional living, financial concerns, family relationships, aging-related changes, and end-of-life issues. This is a full-time position with a day shift schedule.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patient and family needs.
- Assist in discharge planning and offer support during times of loss and grief.
- Collaborate with healthcare professionals to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
