Job Description
We are seeking a compassionate and dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. In this full-time role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address issues related to aging, financial concerns, and end-of-life planning.
- Collaborate with healthcare teams to develop comprehensive care plans.
- Assist in discharge planning and transitions.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
